Patient Info

New patients are asked to bring all pertinent medical records, test results, hard copies of recent breast imaging and a list of current medications and dosages to your initial visit. For your convenience, please download, print and complete your new patient form prior to arrival.

Returning or follow-up patients are also requested to bring any recent breast imaging, as well as an updated medication list to their appointments.

For each office visit, please bring your insurance card, photo identification and co-pay. Each office accepts Visa, American Express, MasterCard, checks and cash.

The Patient Portal is a convenient way to check your account, pay a bill, request a prescription refill and much more. Complete a simple registration process for access.

If you need to cancel an appointment, we ask that you call the office at least 24 hours in advance. This helps us continue to serve all patients in a timely manner.

Our surgeons have privileges at a number of valley hospitals and surgery centers. Prior to your procedure, your surgeon will discuss your options.
